A New Treatment of Lapeyronie’s Disease by Local Injections of Plasma Rich Platelets (PRP) and Hyaluronic Acid. Preliminary Results


Objectives. Evaluate the feasibility and the efficacy of a minimally invasive treatment of LaPeyronie’s disease (PD) using intralesional injections of autologous serum rich in platelets and growth factors (PRP) with hyaluronic acid (HLA).

Patients and methods. Thirteen patients (mean age 57.5y) suffering Peyronie’s disease including 8 with curvatures angles >50° and 5 with deformities (strictures and/or deviation <30° were treated during two months with 4 sessions of injections. A total of 8 ml of whole blood is collected and transferred directly to 2 tubes which content a special gel and HLA. Tubes are centrifuged 5mn at 1500g force and slowly homogenized before being injected, under local anesthesia, in all diseased part of the albuginea. Controls are done one, three and six months (then annually) after completion of the four sessions. Data collected were angle of curvature, size and echographic density of the plaques, International Index of the erectile function-5items (IIEF-5) and the global satisfaction of the patient.

Results. Upon a 9.3 months mean follow up, 10 (77%) of the 13 patients are improved with a mean 30% decrease of the curvature and a diminished density and size of the plaques in 7 cases (53%). IIEF-5 is unchanged or improved in all the cases but one for who it was detected a major caverno-venous leak. The only complication was an important hematoma which disappeared with local ointment. Conclusions. Positive results of this short preliminary series of patients treated by intralesional injections of PRP+HLA in LaPeyronie’s disease are encouraging for the methodology (simplicity) and the practice (inocuity and efficacy). They validate the faisability ad potential efficacy of the autologous cellular therapy. We recommend and shall lead a forthcoming international study to validate this first report.

Platelet Rich Plasma - Future or Trend?


"Chronic complex musculoskeletal injuries that are slow to heal pose challenges to physicians and researchers alike. Orthobiologics is a relatively newer science that involves application of naturally found materials from biological sources (for example, cell-based therapies), and offers exciting new possibilities to promote and accelerate bone and soft tissue healing. Platelet-rich plasma (PRP) is an orthobiologic that has recently gained popularity as an adjuvant treatment for musculoskeletal injuries. It is a volume of fractionated plasma from the patient's own blood that contains platelet concentrate. The platelets contain alpha granules that are rich in several growth factors, such as platelet-derived growth factor, transforming growth factor-β, insulin-like growth factor, vascular endothelial growth factor and epidermal growth factor, which play key roles in tissue repair mechanisms. PRP has found application in diverse surgical fields to enhance bone and soft-tissue healing by placing supra-physiological concentrations of autologous platelets at the site of tissue damage. The relative ease of preparation, applicability in the clinical setting, favorable safety profile and possible beneficial outcome make PRP a promising therapeutic approach for future regenerative treatments. However, there is a large knowledge gap in our understanding of PRPs mechanism of action, which has raised skepticism regarding its potential efficacy and use. Thus, the aim of this review is to describe the various factors proposed to contribute to the biological activity of PRP, and the published pre-clinical and clinical evidence to support it. Additionally, we describe the current techniques and technology for PRP preparation, and review the present shortcomings of this therapy that will need to be overcome if it is to gain broad acceptance."

New and Emerging Strategies in Platelet-Rich Plasma Application in Musculoskeletal Regenerative Procedures: General Overview on Still Open Questions and Outlook


"Despite its pervasive use, the clinical efficacy of platelet-rich plasma (PRP) therapy and the different mechanisms of action have yet to be established. This overview of the literature is focused on the role of PRP in bone, tendon, cartilage, and ligament tissue regeneration considering basic science literature deriving fromin vitro and in vivo studies. Although this work provides evidence that numerous preclinical studies published within the last 10 years showed promising results concerning the application of PRP, many key questions remain unanswered and controversial results have arisen. Additional preclinical studies are needed to define the dosing, timing, and frequency of PRP injections, different techniques for delivery and location of delivery, optimal physiologic conditions for injections, and the concomitant use of recombinant proteins, cytokines, additional growth factors, biological scaffolds, and stems cells to develop optimal treatment protocols that can effectively treat various musculoskeletal conditions."

A Pilot Study of the Effect of Localized Injections of Autologous Platelet Rich Plasma (PRP) for the Treatment of Female Sexual Dysfunction


"Currently, accepted treatments for Female Sexual Dysfunction (FSD) are limited to psychological, behavioral, hormonal and psychopharmacologic interventions. Because of the complex and multifactorial nature of FSD, current therapeutic options may leave a subset of women suffering with sexual dysfunction without clinical improvement. As a simple, safe, and natural alternative therapeutic option for treating female sexual dysfunction, a pilot study was undertaken to test the effect, if any, of vaginal and clitoral injections of autologous Platelet Rich Plasma (PRP) on women desiring treatment for painful intercourse or anorgasmia. Two standardized sexuality tests, the Female Sexual Function Index and the Female Sexual Distress Scale, were administered before and after treatment and were used to measure the response to this therapeutic intervention. Our data indicated some degree of improvement in FSD, including positive changes in isolated sexual difficulties and in the reduction of levels of sexual distress. However, the limited number of participants in this pilot study restricts conclusions. Our initial observations do suggest that further investigation of PRP therapy for the treatment of female sexual dysfunction is indicated."

The role of vacuum pump therapy to mechanically straighten the penis in Peyronie's disease.


"OBJECTIVE: to assess the efficacy of vacuum therapy in mechanically straightening the penile curvature of Peyronie's disease (PD).

PATIENTS AND METHODS: Modelling of the tunica albuginea has been shown to be possible during penile implant surgery and this principle has been applied as an alternative conservative therapy. In all, 31 patients with PD (mean duration 9.9 months; mean age 51 years, range 24-71) completed the study. Over a 12-week period, the patients used a vacuum device (Osbon ErecAid, MediPlus, High Wycombe, UK) for 10 min twice daily. The assessment at study entry and at completion after 12 weeks included the International Index of Erectile Function questionnaire, a perceived pain intensity score, stretched penile length measurement and the angle of penile deformity after an intracavernous injection with prostaglandin E1.

RESULTS: There was a clinically and statistically significant improvement in penile length, angle of curvature and pain after 12 weeks of using the vacuum pump. Of the 31 patients, 21 had a reduction in the angle of curvature by 5-25 degrees, three had worsening of the curvature and there was no change in the remaining seven. The curvature was corrected surgically in 15 patients while the remaining 16 (51%) were satisfied with the outcome.

CONCLUSION: Vacuum therapy can improve or stabilize the curvature of PD, is safe to use in all stages of the disease, and might reduce the number of patients going on to surgery."
